Mikhailov played right wing on the top Soviet line of the 1970s along with Valeri Kharlamov and Vladimir Petrov. In the Soviet hockey league he scored 652 points (428 goals and 224 assists) in 572 games.

On the Soviet national team he played 14 seasons, most of them as captain. He won two Olympic gold medal in 1972 and 1976, and a silver medal in 1980. He won eight IIHF World Championships (1969–71,1973–75,1978,1979). He was named best forward at the IIHF World Championships: 1973 and 1979. And he was the top scorer of the 1974 IIHF World Championships. At the World Championships Boris scored a total of 164 points (98 goals and 66 assist) in 105 games.
